Angioneurotic edema: meaning and warning signs

The modern term avoids implying that the nerves cause the condition. Swelling often appears around the eyelids or lips and may also affect the tongue, hands, feet, genitals, or bowel.

Angioedema involves leakage of fluid into tissue below the skin or mucosal surface. It may accompany itchy hives, but some forms cause swelling without a surface rash. Possible contexts include an allergic-type reaction, a medicine effect, or a less common inherited or acquired pathway.

Not every swollen, red area is angioedema. Cellulitis more often brings progressively painful, warm, inflamed skin and may occur with fever. A clinician uses onset, distribution, itching, pain, medicines, family history, and previous attacks to distinguish the possibilities.

First check for tongue, throat, voice, breathing, or swallowing changes. If none are present, record when and where the swelling began, photograph it, and list recent foods, stings, medicines, infections, and skin exposures. Remove contact with a suspected topical trigger when this can be done safely.

A first or unexplained episode merits medical advice, as does recurrence. Use treatment from a previously agreed plan exactly as directed. Avoid borrowing another person's medicine or assuming that a treatment that helped hives will cover every form of deeper swelling.

Call emergency services immediately for tongue or throat swelling, hoarseness, noisy or difficult breathing, trouble swallowing, blue or grey lips, faintness, or confusion. Airway swelling can progress out of view, so visible facial swelling does not show its full extent.

Seek prompt assessment for rapidly spreading swelling, severe abdominal pain with vomiting, or hot and painful skin with fever. If a prescribed emergency device is part of your action plan, use it as instructed and still obtain emergency help.

Ask what type of angioedema fits the episode, whether a current medicine needs supervised review, and whether allergy or complement testing is relevant. Request clear written instructions covering home treatment, emergency symptoms, and what information to bring after another attack.
