Angioedema without hives: causes and care

Swelling may affect the face, lips, tongue, hands, feet, genitals, or bowel. Episodes without itching or welts can follow a pathway that responds differently from allergic-type swelling, so an accurate history is central to choosing treatment.

Some angioedema is driven by histamine and may occur with or without hives. Other forms involve bradykinin, a chemical that increases fluid leakage from blood vessels; these episodes characteristically lack hives. Certain blood-pressure medicines and inherited or acquired disorders can be relevant.

Abdominal involvement can cause severe cramping, nausea, or vomiting even when the skin looks normal. A clinician distinguishes possible pathways using the timing, family history, medicine list, associated itching, response to past treatment, and pattern of recurrence.

Write down where swelling started, how it progressed, and whether pain, itching, abdominal symptoms, or breathing changes occurred. Bring a complete medicine list, including drugs taken for blood pressure, as well as the age at first episode and any family members with similar swelling.

Seek clinical review for unexplained or recurring attacks. Do not discontinue a prescribed medicine on your own; a prescriber can assess whether it is implicated and select an alternative when appropriate. Treatment used for allergic swelling may not work for every mechanism.

Call emergency services for swelling of the tongue or throat, a new hoarse voice, trouble breathing, difficulty swallowing, faintness, or rapidly worsening facial swelling. Use prescribed emergency medication according to your personal plan, but do not delay the emergency call while waiting for improvement.

Prompt assessment is needed for severe unexplained abdominal pain with swelling, repeated vomiting, or an episode that is progressing. Tell the team clearly that there are no hives and mention any previous diagnosis or family history of angioedema.

Ask whether the clinical pattern suggests histamine- or bradykinin-mediated swelling, whether medicine review or specialist testing is appropriate, and what treatment plan applies to future attacks. Confirm which relatives, if any, should discuss assessment with their own clinician.
