The short answer

The eyelid and nearby loose tissue can become markedly puffy even when the affected area is small. The swelling may occur on one or both sides and can accompany itching or hives, but it may also appear without either.

What swelling under the eye may mean

Angioedema is fluid leakage into deeper layers beneath the skin. Possible triggers include foods, medicines, insect stings, and contact exposures; sometimes no trigger is obvious. Redness, heat, marked tenderness, eye discharge, or fever can point toward another problem, such as infection, rather than uncomplicated angioedema.

A clinician may ask how quickly the puffiness developed, whether it has happened before, and whether the lips, tongue, hands, or feet are also swollen. Photographs taken as the episode changes can help document its pattern.

What to do now

Stop using a newly applied cosmetic or skin product near the eye and avoid rubbing the area. A cool, clean compress may gently ease discomfort. Do not place steroid cream, antihistamine cream, essential oils, or other remedies close to the eye unless a clinician has advised them.

Contact a healthcare professional promptly for a first episode, recurrent episodes, or swelling that continues to spread. Share all recent medicines and supplements, but do not stop a prescribed medicine without clinical advice unless emergency services direct you to do so.

When to seek urgent care

Call emergency services now if swelling involves the tongue, throat, or inside of the mouth, or if there is trouble breathing, noisy breathing, faintness, confusion, or difficulty swallowing. These signs can signal airway involvement.

Urgent assessment is also appropriate for reduced vision, severe eye pain, pain with eye movement, a bulging eye, rapidly increasing redness, or fever. Do not drive yourself if breathing, vision, or alertness is impaired.
