Angioedema During tPA Treatment

Short answer
tPA can rarely be followed by rapid mouth, tongue, or facial angioedema that requires immediate hospital assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

The short answer
In this context, tPA means a clot-dissolving medicine used in time-sensitive care, including some strokes. The reaction is often called orolingual angioedema because it affects tissues around the mouth and tongue.
Why clinicians watch the mouth and airway
Swelling may begin while the medicine is being given or soon afterward. It can be uneven, affecting one side more than the other, and may progress even when there are no hives. Prior use of an ACE inhibitor blood-pressure medicine can increase concern, so the hospital team needs a complete medication history.
Facial weakness from stroke and new tissue swelling are different problems but can coexist. Direct examination of the lips, tongue, voice, swallowing, breathing, and oxygenation helps the team recognise a developing airway reaction.
Speak up as soon as swelling appears
Tell the bedside team immediately about a thick or tingling tongue, new lip swelling, throat tightness, drooling, voice change, or difficulty swallowing. Do not wait for a routine check, eat, drink, or take a home medicine unless hospital staff instruct you.
A relative can help by naming all current medicines and previous swelling episodes. The treating team will decide whether to pause treatment, give medicines for the reaction, increase monitoring, or prepare airway support while continuing stroke care.
When the situation is an emergency
Any new tongue, mouth, or throat swelling during or after tPA is an urgent hospital issue. Noisy or difficult breathing, inability to handle saliva, worsening voice change, reduced alertness, or rapidly expanding swelling demands immediate airway assessment.
After discharge, call UAE emergency services if these symptoms develop or return. Do not drive the affected person, and do not delay because the swelling seems limited to one side.
Questions after recovery
Ask the hospital team to document the suspected reaction clearly, explain which clot-dissolving medicine was used, review any ACE inhibitor, and state what future emergency teams should know. Clarify whether allergy or specialist follow-up is appropriate for the pattern observed.

Questions people ask
Can tPA-related angioedema happen without hives?
Yes. Orolingual swelling after tPA may occur without itchy welts, so the absence of a rash does not remove concern about the airway.
Should tPA be refused because angioedema is possible?
Treatment decisions depend on the emergency, expected benefit, individual risks, and eligibility. The specialist team should explain the decision; suspected swelling during treatment should be reported immediately rather than managed by the patient.
Why does the team ask about blood-pressure medicines?
ACE inhibitor medicines can be relevant to this reaction. Giving the exact medicine name helps clinicians assess risk and respond to swelling.
