Activate local emergency services now for a swollen or tight throat, difficulty breathing or swallowing, noisy breathing, a hoarse or altered voice, tongue swelling, faintness, or rapidly progressing symptoms. Do not drive yourself and do not wait for a rash to appear.

How airway swelling can present

Angioedema can swell deeper tissue in the tongue, mouth, or throat. Because this space is narrow, increasing swelling may restrict airflow. A person may describe throat tightness, difficulty getting words out, drooling, or a sensation that swallowing has become hard.

Hives or facial swelling may accompany throat involvement, but neither is required. A sore or unusual throat has many possible causes; urgency comes from airway warning signs and rapid progression, not from guessing how common angioedema is.

Actions while help is coming

Follow the emergency plan already prescribed for the person. If they have a prescribed adrenaline auto-injector for a severe allergic reaction, use it as instructed and tell the emergency team when it was given. Keep the person accompanied and ready to describe the trigger and symptom onset.

Allow the person to use the position that makes breathing easiest. Do not give food or drink when swallowing is difficult. Antihistamines act too slowly to be the only response to an airway emergency, and oral steroids do not provide immediate airway protection.

Do not delay escalation

Every suspected throat or tongue episode requires urgent emergency assessment, even if symptoms begin to improve. Swelling can change quickly, and clinicians may need to monitor the airway and provide cause-appropriate treatment.

After recovery, arrange follow-up to clarify whether the episode was allergic, medicine-related, inherited, or due to another mechanism. The review should produce a practical recurrence plan, including which medicine to use and when to activate emergency services.
