Angioedema of the Arm

Angioedema usually produces a puffy, tight swelling beneath the skin. The hand, fingers, forearm, or a larger area may be affected, with or without itchy hives. The skin can look normal, pink, or mildly red, and the swelling may feel uncomfortable rather than sharply painful.

Arm swelling alone is not specific to angioedema. Infection, a blood clot, injury, impaired lymph drainage, or reduced blood flow can also enlarge one limb. Heat, marked redness, focal tenderness, fever, a recent injury, or a persistent difference between the arms gives the clinician important diagnostic information.

Remove rings, a watch, and tight sleeves before swelling makes them difficult to take off. Rest the arm in a comfortable raised position and avoid massaging an unexplained swollen area. Note how quickly it appeared, whether hives or itching are present, and any new food, medicine, sting, illness, procedure, or injury.

Use medication only according to an established plan or a clinician's instructions. Oral antihistamines can help some histamine-related episodes but may not work for other types of angioedema or for swelling caused by a clot or infection. Recurrent attacks warrant review of medicines and family history, even if earlier episodes settled.

Contact emergency services immediately if arm swelling occurs with tongue or throat swelling, breathing or swallowing difficulty, voice change, wheezing, faintness, or rapidly spreading symptoms. Follow a prescribed emergency plan while waiting for assistance.

Seek prompt same-day assessment for a suddenly swollen arm, severe pain, weakness or numbness, a cold or discoloured hand, fever, spreading redness, or swelling accompanied by chest pain or shortness of breath. A new isolated episode should also be checked when the cause is unclear, because examination may be needed to distinguish angioedema from circulation problems or infection.

Ask what features point toward angioedema, whether circulation or infection needs evaluation, and which medicines could contribute. Clarify what treatment to use for another episode, whether an emergency medicine is appropriate, and exactly which changes require emergency services rather than a routine appointment.
