The discomfort may build rather than arrive as a sharp pinpoint pain. It can be felt behind the breastbone and may spread to an arm, shoulder, neck, jaw, back, or upper abdomen. Breathlessness, sweating, nausea, or unusual fatigue can accompany it.

What the onset pattern can mean

Angina occurs when part of the heart muscle temporarily receives less oxygen-rich blood than it needs. Physical activity, emotional strain, cold conditions, or a heavy meal can increase the heart's demand and bring on a familiar pattern.

A first episode cannot be safely identified from sensation alone because reflux, muscle pain, lung problems, and urgent heart conditions can also cause chest discomfort. New symptoms need prompt medical assessment, including a description of the trigger, location, spread, and associated symptoms.

What to do when symptoms begin

Stop the activity, sit down, and remain as calm as possible. If a clinician has prescribed medicine for an angina attack, use it exactly as instructed. Do not use another person's medication or drive yourself for urgent help.

Record what you were doing, how the sensation felt, where it travelled, and what relieved it. Even if the episode settles, arrange clinical review if it is new, recurring, or appearing with less effort than before.

When to seek emergency care

Call emergency services for new or severe chest pressure, discomfort that does not settle with rest and the prescribed plan, or symptoms accompanied by marked breathlessness, fainting, cold sweating, confusion, or weakness. A change in a known angina pattern also requires urgent assessment.

While waiting, rest in a safe place, loosen restrictive clothing, and follow the emergency operator's instructions. Do not delay because the sensation is mild or because it resembles indigestion.
