The short answer

Night-time timing does not prove a heart cause or make symptoms harmless. Episodes at rest may occur with unstable angina or coronary artery spasm, while reflux, chest-wall pain and breathing disorders can mimic angina. An examination and appropriate testing are needed to tell them apart.

Why the night-time pattern matters

Angina is often associated with exertion, but reduced coronary blood flow can also cause symptoms at rest. Coronary spasm may follow a recurring time pattern. Lying flat after a meal can bring acid reflux, whereas pain reproduced by pressing or twisting may point toward the chest wall; neither clue is conclusive on its own.

Record whether discomfort wakes you, begins after lying down or follows a late meal. Note pressure, squeezing, burning or sharpness; the exact location; spread to the arm, jaw or back; breathlessness, sweating, nausea or palpitations; and whether sitting up, resting or prescribed medicine changes it.

Safe next actions

Arrange prompt clinical review for repeated night-time chest discomfort even if you feel well by morning. The clinician may check blood pressure, examine the heart and lungs, perform an ECG and decide whether blood tests or longer heart-rhythm monitoring are needed. Testing may be normal between episodes, making a clear symptom diary useful.

Continue prescribed heart medicine as directed and follow your personal angina action plan. Do not take extra doses, borrow another person's glyceryl trinitrate or assume antacids make cardiac assessment unnecessary. Call 998 in the UAE when chest discomfort is new, severe, does not settle with rest or your prescribed plan, or returns in quick succession. Breathlessness, cold sweat, faintness, nausea, unusual weakness, or discomfort spreading to an arm, shoulder, jaw, back or upper abdomen adds urgency.

Do not wait until daylight or drive yourself when these warning signs occur. If a familiar angina pattern starts waking you, happens with less activity, lasts longer or responds differently to medicine, it has changed and needs urgent assessment.

Can reflux and angina feel alike at night?

Yes. Both can cause central chest or upper-abdominal discomfort. Meal timing or relief on sitting up may suggest reflux, but those clues cannot safely exclude a heart cause.

Why might my ECG be normal the next day?

Some ECG changes occur only during an episode, and some heart conditions do not produce obvious changes on a resting trace. Further testing depends on the history and examination.

Is chest pain at rest more concerning?
