What Causes Angina?

The short answer
The usual mechanism is restricted flow through coronary arteries narrowed by fatty deposits. Less common mechanisms include a temporary coronary artery spasm or problems affecting the heart's small blood vessels. Angina describes the resulting symptom pattern, not the underlying artery disorder itself.
The balance between oxygen supply and demand
Physical activity, emotional stress, cold conditions, and heavy meals can make the heart work harder. If blood flow cannot rise enough to match that demand, pressure, tightness, breathlessness, or another angina presentation may follow. Symptoms at rest can occur when blood supply drops without an increase in workload.
High blood pressure can add to the heart's workload and contributes to cardiovascular risk over time. Smoking, abnormal cholesterol, diabetes, kidney disease, and family history may also influence coronary artery disease, although no single factor establishes the diagnosis.
How the cause is assessed
A clinician will ask about the sensation, its location, triggers, duration, spread, associated symptoms, and response to rest or prescribed medicine. The assessment may include a physical examination, an electrocardiogram, blood tests, heart imaging, or a test that evaluates the heart during increased workload.
Treatment is matched to the mechanism and the person's wider health. It may address symptoms, blood pressure, cholesterol, clot formation, smoking, activity, diet, or a significant coronary narrowing. Do not start, stop, or share heart medicine without clinical guidance.
When symptoms are urgent
Seek emergency help for new, severe, or persistent chest pressure; symptoms at rest; fainting; marked breathlessness; cold sweating; or discomfort with nausea or weakness. A familiar pattern that becomes stronger, more frequent, easier to provoke, or slower to settle also needs urgent evaluation.
The urgency comes from the symptom pattern and its change, not from trying to identify the cause at home. Call local emergency services and avoid driving yourself when warning signs occur.
