Chest discomfort should not be assumed to be a normal pregnancy symptom or self-diagnosed as angina. Clinicians consider heart, lung, digestive and musculoskeletal causes while protecting the health of both the pregnant person and baby.

Why pregnancy changes heart care

Pregnancy increases circulating blood volume and makes the heart work harder. Labour and the period after birth bring further circulation changes. A person with existing coronary disease may therefore need coordinated follow-up even when angina has been stable before conception.

Angina can present with pressure, squeezing, tightness or heaviness in the chest, sometimes spreading to an arm, shoulder, neck, jaw or back. Breathlessness, sweating, nausea or light-headedness may accompany it. Pregnancy can cause benign breathlessness or reflux, but these overlaps do not reliably identify the source of chest symptoms.

Coordinate assessment and medicines

Tell your maternity team and heart clinician promptly about new chest discomfort, an established angina diagnosis or previous coronary treatment. Record when symptoms occur, what they feel like, how long they last and whether anything relieves them. An electrocardiogram can be used in pregnancy, and the team can choose further tests according to urgency and likely benefit.

Request a medication review when planning pregnancy, as soon as pregnancy is known, and after delivery. Some heart medicines may need changing, while abruptly stopping others can be hazardous. Do not start, stop or substitute blood-pressure, cholesterol or angina treatment without instructions from a clinician who knows about the pregnancy.

When to seek emergency assessment

Call UAE emergency services for new, severe or persistent chest pressure, symptoms at rest, fainting, marked breathlessness, cold sweating, or discomfort spreading to an arm, jaw or back. Sudden breathlessness with chest pain, coughing blood or collapse also needs immediate help. Use ambulance transport instead of driving.

If angina was diagnosed before pregnancy, treat any departure from the familiar pattern as urgent. Episodes that feel stronger, last longer, occur more often or begin with less activity should be assessed promptly, even if they later settle. Follow the emergency plan agreed with your maternity and cardiac teams.

Plan for pregnancy, birth and recovery

Ask who will coordinate cardiac and maternity care, which medicines are suitable now and during breastfeeding, and which tests are recommended. Discuss safe activity, an emergency symptom plan, the preferred birth setting, pain relief during labour and monitoring after delivery.
