The short answer
Bypass surgery improves blood flow but does not remove the underlying tendency for coronary arteries to narrow. Symptoms may come from changes in a graft or other coronary vessels, although chest pain can also have non-cardiac causes.
Why symptoms may recur
Angina occurs when heart muscle receives insufficient oxygen-rich blood for its workload. After CABG, a graft may narrow or atherosclerosis may progress in vessels that were not bypassed. High blood pressure can also increase the heart's workload.
Pressure, heaviness, squeezing or tightness brought on by walking, hills, cold weather or emotional stress may fit an angina pattern. Pain from the chest wall, lungs or digestive tract can feel similar, so the trigger and associated symptoms matter. An electrocardiogram records electrical activity but may be normal between episodes; further testing depends on the assessment.
What to do about recurrent discomfort
Stop and rest when symptoms begin. Use prescribed fast-acting angina medicine exactly as your care plan directs. If symptoms meet the emergency criteria below, call emergency services instead of driving yourself.
Arrange a review even if episodes settle. Note the location, sensation, trigger, duration, spread to other areas and response to rest or medicine. Keep taking prescribed antiplatelet, cholesterol and blood-pressure medicines unless your clinician changes them; sudden withdrawal may be unsafe.

Call emergency services for chest pressure or tightness that is severe, occurs at rest, is new or clearly worsening, or does not settle according to your prescribed angina action plan. Sweating, nausea, faintness, breathlessness, or pain spreading to the arm, jaw, back or shoulder add urgency.
