Signs beyond breathing
Anaphylaxis does not have to start with wheezing or visible effort to breathe. A serious reaction may present with faintness, confusion, collapse, a weak pulse, pale or clammy skin, or sudden severe drowsiness. Another pattern is rapid involvement of more than one body system after a possible trigger, such as widespread hives with repeated vomiting or severe abdominal cramping.
Throat tightness, trouble swallowing or a new hoarse voice are airway symptoms even when the chest seems clear. Symptoms can evolve, so the absence of respiratory distress at the first check does not show that the reaction will remain stable.
Act without waiting for breathing changes
When the person’s emergency plan indicates anaphylaxis, use their prescribed adrenaline auto-injector immediately and call emergency services. Follow the instructions printed on the device. Do not wait for wheezing, shortness of breath or a low oxygen reading before treating a reaction that already has severe circulation or airway features.
Lay the person flat with raised legs if this is comfortable and safe. Place someone who is unconscious but breathing on their side, and avoid allowing them to stand or walk. Keep checking whether they can speak clearly, remain alert and breathe comfortably. If the action plan calls for another dose because serious symptoms persist, give it with the next available device as directed.
Features needing urgent care
Seek emergency care for collapse, fainting, confusion, pronounced weakness, throat tightness, tongue swelling, voice change, blue or grey colour, or rapidly worsening symptoms after a suspected allergen. Emergency help is also warranted when a known allergy is followed by the severe features described in the person’s action plan. Improvement after adrenaline does not remove the need for assessment because symptoms may recur.
Review your action plan
Ask your clinician which non-breathing symptoms should prompt adrenaline in your case and how the suspected trigger can be assessed. Have them demonstrate positioning and auto-injector technique, then practise with a trainer device if available. Clarify how family, carers or colleagues should recognise circulation changes when you cannot explain what you feel.
