Why anaphylaxis affects breathing

During anaphylaxis, swelling can narrow the upper airway while tightening of the lower airways can cause wheezing and difficult breathing. Circulation can also fail, leaving a person weak, dizzy, confused, or unconscious. These effects may progress rapidly and may occur without hives.

Shortness of breath has many other causes, including asthma, heart conditions, anaemia, and lung infections. The direction that matters is that anaphylaxis commonly presents with acute breathing difficulty; breathlessness by itself does not establish anaphylaxis. Sudden onset after food, medicine, a sting, or another known trigger strengthens concern when allergic features are also present.

How to respond

Use the person’s adrenaline auto-injector in the outer thigh at once and call UAE emergency services. Follow the device and personal action-plan instructions. Keep the person lying flat with raised legs when possible. If this makes breathing harder, supported sitting with legs extended is reasonable, but the person should not stand or walk.

An asthma reliever may be used for wheeze after adrenaline if the person’s plan recommends it, but it cannot treat throat swelling or circulatory collapse. If symptoms continue, the action plan may direct use of another adrenaline device while the ambulance is on its way.

Breathing signs that need emergency care

Call urgently for noisy breathing, inability to speak normally, throat closure, blue or grey lips, severe wheezing, gasping, marked drowsiness, faintness, or collapse. Do not drive yourself if symptoms are severe. Emergency assessment remains necessary after improvement because breathing or circulation problems can return.
