Anaphylaxis and penicillin allergy: what you should know

Do not wait for a rash to appear. Sudden breathing difficulty, throat or tongue swelling, faintness, collapse, or rapidly worsening symptoms after penicillin are enough to treat the situation as an emergency.

Anaphylaxis is a severe allergic reaction that can affect the airway, breathing, circulation, skin, or digestive system. A person with anaphylaxis may develop wheezing, a hoarse voice, swelling, widespread hives, vomiting, dizziness, or loss of consciousness. Skin changes can be absent.

Not every symptom reported after penicillin is an allergy. Nausea, diarrhoea, an infection-related rash, and some delayed rashes may have other explanations. The timing, exact medicine, symptoms, and treatment received help an allergy specialist judge what happened.

If anaphylaxis is suspected, use the person’s adrenaline auto-injector without delay, following its instructions, and call UAE emergency services. Keep the person lying flat with legs raised if possible; if breathing is difficult, allow them to sit with legs extended. Do not let them stand or walk. A second device may be needed if the emergency plan directs it and symptoms continue.

After recovery, record the penicillin name, dose, route, timing, symptoms, and emergency treatment. Avoid penicillin until a clinician has reviewed the event. Do not test the allergy by taking another dose yourself.

Call emergency services immediately for difficult or noisy breathing, wheezing, throat tightness, tongue swelling, trouble speaking, blue or pale colour, confusion, faintness, or collapse after penicillin. Repeated vomiting with other allergic features also needs urgent assessment. Medical observation is needed after adrenaline because symptoms can return.

Ask which medicines to avoid, whether related antibiotics are suitable, how the allergy should appear in medical records, and whether referral for structured allergy testing is appropriate. Confirm who should carry an adrenaline auto-injector and request a written emergency plan if one is prescribed.
