Anaphylaxis and fever: what you should know

Do not use the presence or absence of fever to decide whether a reaction is an emergency. Sudden breathing difficulty, throat or tongue swelling, faintness, collapse, or rapidly worsening symptoms need immediate action.

Anaphylaxis is a rapid, severe allergic reaction that can affect breathing, circulation, skin, or the digestive system. Flushing and feeling hot may occur during a reaction, but a measured raised temperature can point to a separate illness or to the underlying problem for which a medicine was taken. Some medicine reactions can include both fever and allergic features, so the timing of every symptom matters.

Skin changes are not required for anaphylaxis. A person can have dangerous airway or circulation symptoms without hives, and fever does not make those warning signs less urgent.

Use an adrenaline auto-injector immediately if anaphylaxis is suspected and one is available, following the device instructions and the person’s action plan. Call UAE ambulance services on 998, say that you suspect anaphylaxis, and arrange emergency assessment even if symptoms improve. Do not delay adrenaline while checking a temperature or waiting for an antihistamine to work.

Keep the person lying down unless breathing is easier while sitting with legs extended. Avoid letting them stand or walk. If they are unconscious but breathing, place them on their side; if they are not breathing normally, begin CPR if you know how and follow the emergency call handler’s instructions.

Treat sudden difficulty breathing, noisy breathing, wheezing, swelling of the tongue or throat, trouble swallowing, severe dizziness, confusion, fainting, or collapse as an emergency. Rapidly worsening symptoms affecting more than one body system after possible allergen exposure also require urgent help.

Emergency evaluation remains necessary after adrenaline because symptoms can return after seeming to settle. A fever that persists after the allergic symptoms have resolved also deserves clinical assessment, especially with a new rash, severe headache, stiff neck, repeated vomiting, unusual sleepiness, or signs of dehydration.

Ask what trigger is suspected, whether a prescribed medicine should be avoided, and whether referral for allergy testing is appropriate. Clarify whether you need adrenaline auto-injectors, where to carry them, how to check their expiry dates, and who around you should learn to use them.

Bring a timeline of foods, medicines, insect stings, activities, temperature readings, and symptom onset. Photographs of a rash and details of any treatment given can help the clinician distinguish an allergic reaction from an infection or another cause.
