Anaphylaxis and Benadryl: why antihistamines are not enough

If anaphylaxis is suspected, use the person's prescribed adrenaline (epinephrine) auto-injector immediately and contact local emergency services; do not wait for an antihistamine to work.

Benadryl is a brand name commonly associated with the antihistamine diphenhydramine, although product ingredients can differ by country. Antihistamines block effects of histamine and may reduce itching or hives. They do not rapidly reverse throat swelling, severe breathing problems, or the dangerous fall in blood pressure that can occur in anaphylaxis.

Drowsiness from diphenhydramine can also make it harder to judge whether confusion or reduced alertness is worsening. Check the active ingredient on the package and reserve any antihistamine for the supporting role specified by a clinician or allergy action plan.

Give the prescribed adrenaline auto-injector in the outer thigh as directed, then call local emergency services and report suspected anaphylaxis. Use another prescribed injector only according to the person's action plan or emergency guidance if symptoms continue. An antihistamine can be considered only after adrenaline when the plan or a clinician advises it, and it must not delay transport.

Position the person flat with raised legs when breathing is comfortable. Let them sit supported with legs extended if lying flat makes breathing harder; place a pregnant person on their left side. Prevent standing or walking. Start CPR if the person is unresponsive and not breathing normally, provided you know how.

Call for emergency help for throat or tongue swelling, a hoarse voice, wheezing, difficult or noisy breathing, sudden dizziness, confusion, fainting, or collapse after possible allergen contact. Anaphylaxis can occur without a rash, and temporary improvement after medicine does not remove the need for assessment.

Emergency clinicians can support the airway and circulation and monitor for returning symptoms. Take the suspected food or medicine packaging when safe, along with the used auto-injector, but never delay departure to collect items.

Ask which symptoms should trigger immediate adrenaline, whether an antihistamine has any place in your personal action plan, and which active ingredient and formulation are suitable for you. Review conditions and medicines that could affect emergency treatment, and practise the injector technique with a trainer device.

Confirm how to store and replace auto-injectors, who should know where they are kept, and whether allergy testing could help identify the trigger. A written plan prevents uncertainty about medicine order during a future reaction.
