What should you know about anaemic heart failure?

The phrase does not describe a single separate diagnosis. It generally refers to heart failure complicated by anaemia, or significant anaemia contributing to cardiovascular strain. Treatment depends on why each condition developed.

Heart failure may present with breathlessness, swollen ankles, fatigue, reduced exercise capacity or difficulty lying flat. Anaemia may also present with fatigue, palpitations, dizziness or exertional breathlessness. Their overlapping symptoms make examination and testing important.

Assessment may include a full blood count, iron studies, kidney function, medicine review, heart tracing and evaluation of heart structure or pumping function. Possible reasons for anaemia include iron deficiency, bleeding, kidney disease, nutritional deficiency or inflammation associated with chronic illness.

Take heart and anaemia medicines exactly as directed, and bring an updated medicine list to reviews. Do not start an iron product unless iron deficiency has been assessed; iron will not correct every type of anaemia and can interact with the timing of other medicines.

Track changes in breathing, ankle swelling, energy, appetite and body weight according to the plan provided by your heart-failure team. Sudden changes can be more useful than a single isolated observation. Follow personalised advice on fluids and salt rather than imposing a restriction yourself.

Call emergency services for severe breathlessness, chest pressure or pain, fainting, blue or grey lips, new confusion, or coughing up pink frothy fluid. These signs need immediate assessment regardless of whether anaemia is known.

Contact the clinical team promptly for increasing ankle or abdominal swelling, breathlessness that is worse than usual, new difficulty sleeping flat, persistent palpitations, black or bloody stools, or a noticeable fall in day-to-day stamina.

Ask what is causing the anaemia, whether iron stores and kidney function have been checked, how improvement will be monitored and which symptom changes should trigger a call. Clarify whether any current medicine could contribute to bleeding or affect blood results.
