How infection happens

The parasite passes in stool and can spread when microscopic contamination reaches the mouth. This may happen through unsafe water, food handled with poor hand hygiene, unwashed produce, or sexual contact involving faecal exposure. The organism can remain in the bowel without causing symptoms, while in some people it invades the bowel lining and causes amoebic colitis. Less commonly, it travels through the bloodstream and forms an abscess in the liver.

Recent travel or residence in a place with inadequate sanitation can increase exposure, but infection can occur elsewhere as well. Amoebiasis cannot be identified from exposure history alone because other intestinal infections can cause similar problems.

Practical next steps

If you have ongoing loose stools, abdominal cramps, or blood or mucus in stool, arrange a clinical assessment. A clinician may request stool tests and sometimes other tests to distinguish Entamoeba histolytica from harmless related organisms. Do not start leftover antibiotics or anti-diarrhoeal medicine without advice, especially when stool contains blood.

Reduce further spread by washing hands with soap after using the toilet and before preparing food. Use safely treated water, wash produce carefully, and avoid preparing meals for others while you have diarrhoea. Treatment should be selected by a clinician because medicine may be needed both for invasive infection and for parasites remaining inside the bowel.

When to seek care

Seek urgent medical care for severe or worsening abdominal pain, repeated vomiting, marked weakness, fainting, confusion, very little urine, or an inability to keep fluids down. Blood in the stool, high fever, or pain in the upper right abdomen also warrants prompt assessment. Babies, older adults, pregnant people, and anyone with weakened immunity should be assessed early when diarrhoea is persistent or hydration is becoming difficult.
