Understanding an amnio dye test

When it is difficult to tell whether fluid from the vagina is amniotic fluid, a specialist may discuss a dye-based procedure. Under ultrasound guidance, fluid containing a suitable medical dye is introduced into the amniotic sac through a needle. The team then checks whether coloured fluid appears in the vagina, which can support a diagnosis of ruptured membranes. Local technique and the dye used may differ.

This procedure has a different purpose from laboratory testing of amniotic fluid for chromosome or inherited conditions. The wording may also be used loosely in conversation, so the referral or consent form should state what is being placed or sampled, why it is needed and what the possible result means.

Ask whether the aim is to check for membrane rupture or to collect fluid for another analysis. Tell the clinician about allergies, medicines that affect clotting, bleeding conditions, previous reactions to dyes and your blood group if known. Request the exact name of the substance, how ultrasound will be used, what monitoring is planned and whether you need someone to accompany you home.

Follow the maternity unit's instructions about food, drink, bladder filling and activity afterward. If you are currently leaking fluid, note its colour, smell and approximate timing without inserting anything into the vagina unless a clinician has advised it.

Contact your maternity unit promptly for ongoing watery leakage, vaginal bleeding, fever, chills, increasing abdominal or pelvic pain, regular contractions, unpleasant-smelling discharge, or reduced fetal movement once movement is familiar. Seek urgent help for heavy bleeding, severe pain, fainting, breathing difficulty or rapidly worsening illness. These warning signs require assessment whether they occur before or after a dye-based procedure.

Useful questions include: What diagnosis are you checking? Are there other ways to assess the fluid leakage? Which dye and route will you use? How will the result change my care? Also establish where to go if leakage, contractions or fever develops after you leave and how follow-up will be arranged if the result does not settle the diagnosis.
