Alternatives to hysterectomy

An alternative is only suitable when it safely addresses the cause of bleeding or pain and matches your plans for pregnancy. Hysterectomy may still be recommended when a less invasive approach is unlikely to control disease or when cancer is present.

Heavy periods may be managed with non-hormonal tablets or hormonal contraception. Endometrial ablation treats the womb lining and is intended for people who do not plan a future pregnancy. Fibroids may be treated by removing selected fibroids, reducing their blood supply, or using other targeted techniques. Endometriosis surgery can remove visible disease without removing the womb.

The diagnosis changes the choice. Adenomyosis grows within the womb muscle and may respond differently from a fibroid. Endometrial hyperplasia requires treatment based on its cell changes. Cervical cancer and womb cancer need specialist planning; symptom-control treatments are not substitutes for cancer care.

Confirm the working diagnosis and ask whether imaging, examination, or a sample from the womb lining is needed before deciding. Explain how bleeding or pelvic pain limits work, sleep, exercise, sex, and caring responsibilities. Mention previous medicines, procedures, anaemia, other health conditions, and whether preserving fertility matters to you.

Compare expected symptom relief, recovery, repeat-treatment possibility, effects on fertility, and what happens if the first option does not work. A second specialist opinion can help when the diagnosis is uncertain or the choices carry very different consequences.

Seek urgent help for bleeding that rapidly soaks through protection, fainting, severe weakness, chest pain, breathlessness at rest, sudden intense pelvic pain, fever with pelvic pain, or possible pregnancy with pain or bleeding. These signs need assessment regardless of whether surgery has been discussed.

Arrange a timely appointment if periods are increasingly heavy, bleeding occurs after sex or after menopause, pain is worsening, or tiredness and breathlessness suggest iron deficiency anaemia.

Ask what is causing your symptoms, which options preserve the womb, whether each option preserves the ability to carry a pregnancy, what recovery involves, and what would trigger reconsideration of hysterectomy. Also ask whether the ovaries would be affected by any proposed surgery, because hysterectomy and ovary removal are separate decisions.
