Alternatives to a Pap smear

A Pap smear examines cervical cells for abnormal changes, while an HPV test looks for high-risk human papillomavirus types linked to those changes. Some services offer HPV self-collection for eligible people, although a clinician-collected cervical sample may still be needed after certain results.

Primary HPV testing can identify whether a high-risk viral type is present before cell changes are assessed. Depending on the result and programme, the same clinician-collected sample may then be checked for abnormal cells, or a follow-up sample may be requested. A self-collected vaginal sample generally tests for HPV; it does not collect cervical cells for a Pap smear.

Colposcopy is not a routine substitute for screening. It is a closer examination of the cervix used when a screening result or clinical finding needs further evaluation, and a small tissue sample may be taken if an area looks abnormal.

Contact a gynaecology or screening service and explain whether your concern is discomfort, privacy, trauma, disability, pregnancy, a previous abnormal result or difficulty attending. This lets the service identify a valid screening option and plan practical support such as a smaller speculum, a chosen clinician, extra time or a support person.

Ask whether primary HPV testing is offered, whether you qualify for self-collection, where the sample can be taken, and what follow-up each possible result requires. Do not substitute a home product unless it is part of a recognised clinical pathway that can return a result and arrange follow-up.

Screening is intended for people without concerning cervical symptoms. Arrange a clinical assessment for bleeding after sex, bleeding between periods, bleeding after menopause, unusual persistent discharge, or ongoing pelvic pain; a screening test alone may not evaluate the cause. Seek urgent care for heavy bleeding, fainting, severe pelvic pain, fever with worsening illness, or possible pregnancy with pain and bleeding.

Ask which screening method applies to you, whether your previous results change the plan, and what a positive HPV result would lead to. If the examination is the barrier, describe what would make it manageable and ask which adjustments the clinic can provide.
