High ALT or SGOT on a blood test

ALT is concentrated mainly in the liver. SGOT is an older name for AST, an enzyme present in the liver as well as muscle and other tissues. A report that lists both is describing separate measurements, not two names for the same test.

The laboratory flags a result when it falls outside its own reference range. Clinicians compare ALT with AST and other results such as bilirubin, alkaline phosphatase, albumin and sometimes creatine kinase. The combination, your symptoms and changes over time are more useful than either enzyme in isolation.

Liver inflammation, fatty liver change, alcohol, medicines and supplements can raise these enzymes. Because AST also comes from muscle, vigorous exercise or a muscle injury may contribute to an elevated SGOT result. Blood tests can guide the investigation, but they cannot establish the cause without a medical history and sometimes further testing.

Check whether the report says ALT, AST or SGOT and note which values were flagged. Tell the clinician about recent illness, heavy exercise, alcohol, muscle pain, prescription medicines, pain relievers, herbal remedies and sports supplements. Continue prescribed medicines unless the prescriber advises a change.

Follow-up may include a repeat sample after avoiding a suspected temporary trigger, additional liver tests, a muscle enzyme test, infection testing or imaging. The appropriate choice depends on the pattern rather than a single universal checklist.

Get urgent medical help for yellow skin or eyes, confusion, unusual sleepiness, persistent vomiting, severe abdominal pain, unexpected bleeding, very dark urine or rapidly worsening weakness. Severe muscle pain with dark urine also warrants prompt assessment. An abnormal result without these symptoms still deserves planned follow-up.

Ask whether SGOT on your report means AST, whether ALT and AST rose together, and whether other liver or muscle markers were affected. Also clarify when to repeat the test and which medicines, supplements or activities could influence the next sample.
