High ALT on a liver function test: what it means and what to do

Your clinician interprets the value alongside the laboratory reference range, other liver blood results, medicines, alcohol use, health conditions and any symptoms. One abnormal result may lead to a repeat blood test or further assessment rather than an immediate diagnosis.

ALT, or alanine aminotransferase, is an enzyme found mainly inside liver cells. When those cells are irritated or damaged, ALT can enter the bloodstream. The pattern across ALT and the other components of a liver function test can be more informative than ALT alone.

Possible explanations include fatty change in the liver, alcohol-related injury, viral hepatitis, and effects from prescribed medicines, non-prescription painkillers or supplements. Strenuous exercise and muscle injury may also influence some blood enzyme results. A clinician uses your history and, when appropriate, repeat tests, hepatitis testing or imaging to narrow the cause.

Arrange follow-up with the clinician who requested the test, even if you feel well. Bring a complete list of prescriptions, over-the-counter products, vitamins, herbal products and gym supplements, and describe your usual alcohol intake honestly. Do not stop a prescribed medicine without clinical advice because suddenly stopping may cause harm.

Until the result is reviewed, avoid starting new supplements or taking more than the labelled dose of any medicine. Your clinician may compare earlier results, repeat the liver panel after an appropriate interval, or request other tests based on the overall pattern.

Seek urgent medical assessment if you develop yellow skin or eyes, confusion, unusual drowsiness, vomiting blood, black stools, severe or worsening upper abdominal pain, a swollen abdomen, or repeated vomiting that prevents you from keeping fluids down. These symptoms matter because they can signal impaired liver function, bleeding or another acute illness.

Contact a clinician promptly for dark urine, very pale stools, persistent itching, fever with abdominal discomfort, or a suspected reaction to a medicine or supplement. If you may have taken too much paracetamol, obtain emergency advice immediately rather than waiting for symptoms or another blood test.

Ask how far the ALT is from your laboratory's reference range, whether other liver results follow the same pattern, and whether a repeat test is needed. You can also ask whether any medicine, supplement, recent illness, alcohol exposure or metabolic condition could fit the result, and which symptoms should change the timing of follow-up.
