Alopecia Areata and a Painful Scalp

Some people notice tingling, itching, tenderness, or a burning sensation around an active patch. Pain can also come from scratching, tight hairstyles, sunburn, inflamed follicles, infection, or a reaction to a scalp product, so the symptom deserves its own evaluation.

Alopecia areata is an immune-mediated condition in which inflammation around hair follicles interrupts hair growth without usually destroying the follicles. The exposed skin often looks smooth and normal. Marked redness, crusting, pus, scaling, blisters, or broken hairs are not typical features of a simple smooth patch and can suggest an additional scalp problem.

A clinician may examine the scalp closely, ask when the discomfort began, and check whether the hair loss is patchy, diffuse, or accompanied by skin change. This helps distinguish alopecia areata from fungal infection, traction, contact dermatitis, folliculitis, and scarring disorders.

Pause new dyes, fragranced scalp products, harsh exfoliants, and tight styles until the cause of pain is clearer. Wash gently, avoid picking, shield bare patches from strong sunlight with a hat, and note whether discomfort follows a product or hairstyle. Photographs taken in similar lighting can help show whether redness or the patch boundary is changing.

Do not start leftover steroid creams or someone else’s medicine. Treatment depends on the diagnosis, the amount and location of hair loss, other health conditions, and the effect on daily life.

Arrange a dermatology or primary-care assessment if pain persists, returns, or accompanies new hair loss. Seek prompt care for rapidly increasing redness or swelling, drainage, fever, spreading tenderness, blisters, or pain severe enough to disturb sleep. Sudden eyebrow or eyelash loss, nail changes, or extensive shedding also warrants evaluation.
