Is alopecia areata permanent?

Its course is difficult to forecast for one person. Some patches fill in without treatment, some respond during treatment, and others remain active. Regrowth does not prevent a later episode in the same place or elsewhere.

In alopecia areata, immune activity interrupts the follicle's growth cycle without typically destroying the follicle. New hairs may first be fine, short or lighter than surrounding hair and can change as growth continues.

Permanence is a different concern when follicles have been scarred. A shiny surface, absent follicle openings, burning, pain, pustules or heavy scale is not something to judge from photographs alone; it requires examination for scarring alopecia, infection or another scalp disorder.

Have the diagnosis confirmed before assuming the loss cannot recover. Protect exposed scalp from sun and cold, handle nearby hair gently, and avoid irritating products on inflamed skin. Photographs taken in consistent conditions can show subtle regrowth more reliably than daily inspection.

Treatment aims to reduce immune disruption and encourage growth; it does not promise that future patches will never appear. Depending on location and extent, a dermatologist may discuss topical medicine, injections into patches or a systemic option, balancing likely benefit with side effects and monitoring.

Seek assessment for a first bald patch, continued expansion, widespread shedding, or new eyebrow and eyelash loss. A prompt visit is appropriate if the scalp becomes painful, red, swollen, crusted, draining or distinctly scaly because those features can signal a different process requiring specific care.

Return during treatment if side effects develop or the agreed review point passes without the expected change. Tell the care team when uncertainty about regrowth is causing persistent anxiety or affecting everyday activities.
