Alopecia areata at the frontal hairline

Alopecia areata may form a smooth focal patch at the front or a band-like pattern near the scalp margin. Location alone cannot establish the diagnosis.

With alopecia areata, the exposed surface often remains smooth and the follicle openings are still visible. Short tapered hairs may be seen near an active border, and separate patches can occur elsewhere on the scalp, beard or brows. Nail pitting can accompany the hair changes.

Gradual symmetrical recession may fit pattern hair loss, while loss where tight braids, extensions or headwear pull can suggest traction. Redness, scale, burning, bumps or a shiny surface with missing follicle openings raises concern for an inflammatory or scarring process.

Loosen hairstyles that place tension on the front and avoid adhesives, harsh chemicals or repeated heat over tender skin. Take clear photographs of the centre and temples with the hair positioned the same way, and note itching, burning, shedding elsewhere or recent treatment.

A dermatologist may inspect the area with magnification and examine the rest of the scalp, brows, nails and skin. Limited alopecia areata may be treated with topical corticosteroids or injections into the affected skin. A different diagnosis calls for different treatment, especially when scarring is possible.

Seek prompt dermatology care if the hairline is painful, burning, markedly itchy, red, scaly or developing a shiny scar-like surface. These features may accompany conditions capable of lasting follicle damage, so urgency comes from the scalp findings rather than from guessing how common a diagnosis is.

Arrange a standard appointment for a smooth patch that enlarges, new patches elsewhere, eyebrow loss or uncertainty about the pattern. Sudden diffuse shedding also merits review because it may represent a separate form of hair loss.

Ask whether follicle openings are preserved, whether there are signs of traction or inflammation, and whether further testing is needed. Clarify where any prescribed product should be applied, how to protect nearby facial skin and what change would justify an earlier follow-up.
