Exclamation-mark hairs in alopecia areata

The name describes their shape: the hair is thinner where it emerges and broader farther from the skin. They may be difficult to see without magnification and are only one part of the diagnosis.

Inflammation around a growing follicle can weaken production near the root. The fragile shaft narrows and then breaks, leaving a short tapered remnant. A cluster near the border of a smooth bald area can help a dermatologist recognise current activity in alopecia areata.

Their absence does not rule out the condition, and seeing one short hair at home does not establish it. Ordinary breakage, traction, fungal infection, and hair-pulling can also leave hairs of different lengths. Scalp scaling, redness, black dots, the distribution of broken hairs, and follicle appearance help separate these causes.

Avoid plucking the hairs or repeatedly testing the patch, because that removes clues and can irritate the skin. Photograph the area in clear light, note whether the border is enlarging, and check gently for other smooth patches without scraping or picking.

A dermatologist can inspect the scalp with dermoscopy, looking at shaft width, broken ends, follicle openings, and nearby regrowth. The examination also covers other scalp areas, nails, eyebrows, eyelashes, and relevant medical history. Management is based on the complete pattern and the extent and impact of loss.

Arrange a dermatology visit when a smooth patch is new, enlarging, or accompanied by additional bald areas. Seek prompt care for pain, swelling, pus, heavy crust, fever, or tender neck glands, because infection may produce broken hairs and needs different treatment.

Shiny skin, loss of visible follicle openings, or persistent burning also requires timely review for possible scarring hair loss. If eyelashes are affected and the eyes become persistently irritated, ask for assessment of both the hair loss and eye-surface symptoms.

Ask whether the short hairs are tapered at the root or simply broken, whether magnification shows activity elsewhere, and what alternative diagnoses were considered. You may also ask how the border will be monitored and which signs would show regrowth rather than continued breakage.
