A bald spot and alopecia areata

The patch may be round or oval, with skin that is not heavily scaled or scarred. Short tapered hairs can occur near an active edge. Appearance alone is not enough to diagnose every patch.

A dermatologist looks closely at follicle openings, broken hairs, scaling, redness and the border of the area. Alopecia areata interrupts growth without destroying the follicle, so the affected surface is generally smooth and regrowth remains possible, although timing and completeness vary.

Visible scale or many broken hairs can occur with a fungal scalp infection. Repeated pulling or tight hairstyles produce another pattern, while shiny skin and loss of follicle openings raise concern for scarring hair loss. These distinctions matter because each cause needs different management.

Take a baseline photograph with the hair parted in the same way and avoid repeatedly tugging hairs to test the edge. Use gentle washing, reduce traction from tight styling, and protect exposed scalp from strong sun with a hat or suitable sunscreen.

Schedule an examination rather than treating the spot with leftover creams. If alopecia areata is confirmed, management may range from observation to topical corticosteroids or steroid injections, depending on activity, location, extent and personal circumstances.

Arrange care if the spot enlarges, more patches appear, or loss extends to the beard, eyebrows or eyelashes. A review is also appropriate when shedding is diffuse, the nails develop pits, or the patch keeps returning.

Pain, crusting, pus, marked itch, swollen glands, prominent scale or a shiny scar-like surface calls for prompt assessment. Those findings can signal infection or an inflammatory scarring process rather than a straightforward alopecia areata patch.

Ask which features identify the cause, whether dermoscopy or testing is needed, and whether the follicle openings are preserved. It is also useful to clarify how frequently to photograph the site and what change should prompt an earlier follow-up.
