Bone metastases can stimulate bone breakdown and rebuilding, which may release more alkaline phosphatase into the blood. Some people with bone metastases do not have an elevated result, so a value within the laboratory range does not exclude them.

A raised value may instead relate to liver or bile-duct problems, healing fractures, or other bone conditions. The pattern over time and related tests can help identify the likely source. Imaging is needed when the history, examination, or test pattern raises concern about cancer in bone.

Discuss whether the result is new, changing, or accompanied by abnormalities in liver-related tests or calcium. Tell the team about recent fractures, bone disease, medicines, and the location and timing of any pain. Do not use an isolated alkaline phosphatase value to judge whether treatment is working.

If bone metastases are confirmed, care can address the cancer and protect daily function. Depending on the situation, options may include systemic cancer therapy, bone-strengthening treatment, radiotherapy to a painful area, and a structured pain-management plan.

Seek urgent assessment for new severe back or neck pain, especially with leg weakness, numbness, difficulty walking, or loss of bladder or bowel control. These can signal pressure on the spinal cord and should not wait for a routine appointment.

Sudden intense pain after a minor movement, inability to bear weight, or a visibly changed limb can indicate a fracture. Confusion, marked drowsiness, persistent vomiting, or unusual thirst also warrants prompt medical advice because calcium levels may need checking.

Ask whether the alkaline phosphatase appears to come from bone or liver, which comparison tests are useful, and whether imaging is indicated. If bone metastases are present, clarify how fracture risk will be assessed, what pain relief is available, and whether dental review is needed before bone-strengthening medicine.
