Redness, itching, burning, tenderness, flaking, crusting, or mild swelling can develop on the treatment site. The prescriber uses both the skin response and your comfort to decide whether the schedule should continue, pause, or change.

Imiquimod activates immune signalling in the treated skin, so some inflammation is part of its local effect. A stronger visible reaction does not necessarily mean better treatment, and little redness does not by itself prove that treatment has failed. Reactions may extend slightly beyond the application spot if cream spreads, while sensitive sites can become uncomfortable more quickly.

Apply only the prescribed amount to the defined area and wash it off at the instructed time. Wash your hands afterward and avoid the eyes, lips, nostrils, and internal genital surfaces unless your clinician has given site-specific directions. Do not add acids, retinoids, scrubs, waxing, or other irritating products to the area without checking first. If discomfort is increasing, contact the prescriber before changing the schedule or using a steroid cream.

Stop treatment and seek prompt guidance for extensive raw skin, intense pain, heavy swelling, pus, spreading redness, difficulty passing urine after genital application, or flu-like symptoms that make normal activity difficult. Breathing trouble, swelling of the face or throat, fainting, or widespread blistering requires urgent medical help. If cream enters an eye, rinse with plenty of clean water and obtain advice if irritation persists or vision changes.

Show the clinician how far the redness extends and describe pain, sleep disruption, discharge, or whole-body symptoms. Ask whether to pause applications, whether a bland moisturiser or dressing is appropriate, and how the treatment site will be checked after the inflammation settles.
