Aldactone in heart failure: uses and monitoring

Aldactone is a brand of spironolactone, a mineralocorticoid receptor antagonist and potassium-sparing diuretic. It is prescribed as one part of a broader heart failure plan rather than as a substitute for other recommended medicines.

Aldosterone encourages the body to retain sodium and water and contributes to changes in heart tissue. Spironolactone blocks its action. This can help the body release excess salt and fluid, easing congestion such as swollen ankles or breathlessness, while also addressing hormone effects involved in heart failure.

Suitability depends on the type and severity of heart failure, kidney function, potassium level, blood pressure, and the rest of the medicine regimen. It is not appropriate for every person with heart failure.

Take the prescribed dose at the same time each day and follow instructions about missed doses. Do not double a dose or stop treatment without speaking to the prescriber. Blood tests are used before and after starting or changing treatment to check potassium and kidney function.

Tell the clinician about all prescriptions, non-prescription painkillers, supplements, and salt substitutes. Potassium supplements and potassium-based salt substitutes may raise potassium too far, while some anti-inflammatory painkillers can worsen kidney function or fluid retention. Report vomiting, diarrhoea, or poor fluid intake because illness may change how medicines should be managed.

Get urgent advice for new muscle weakness, marked drowsiness, an irregular heartbeat, fainting, or very little urine; these can accompany potassium or kidney problems. Contact the heart failure team promptly if swelling, breathlessness, or weight rises noticeably, as this may mean fluid balance or treatment needs review.

Call emergency services for severe breathing difficulty, chest pain, collapse, or blue or grey lips. Breast tenderness, breast enlargement, menstrual changes, and sexual difficulties are recognised adverse effects to discuss at a routine review unless severe or distressing.
