Aldactone in gender-affirming care

Aldactone is a brand of spironolactone. In gender-affirming treatment, its anti-androgen action may complement oestrogen or be considered in another individualized plan. It is not a feminizing hormone itself, and no single regimen suits every transgender or gender-diverse person.

Reduced androgen activity may support less oily skin, slower body-hair development, fewer spontaneous erections or changes in libido. Responses and timing vary, and established facial hair or voice changes may need separate approaches if they are treatment goals.

Spironolactone also makes the kidneys release salt and water while retaining potassium. More frequent urination, light-headedness and lower blood pressure can therefore occur alongside the intended anti-androgen effect.

Discuss the changes you want, fertility wishes, sexual function and any concerns about mood or body changes with a clinician experienced in gender-affirming care. Fertility preservation is best considered before treatments that may affect sperm production.

Share any history of kidney or adrenal disease, low blood pressure, and all medicines or supplements. Keep appointments for review and requested blood tests, particularly checks of potassium and kidney function. Never use another person's tablets or adjust the dose to accelerate changes.

Get urgent medical help for fainting, breathing difficulty, facial or throat swelling, severe confusion, pronounced weakness, or a slow or irregular heartbeat. A significant fluid, potassium or allergic problem needs prompt assessment.

Call the prescribing team soon if ongoing dizziness, reduced urination, vomiting or diarrhoea makes it hard to stay hydrated. New symptoms should be assessed without assuming they are an expected part of gender-affirming treatment.
