Alcohol urine tests: what the result can show

The meaning depends on what the laboratory measured, when the sample was collected, the test threshold, and the clinical or administrative reason for testing. A urine result should therefore be interpreted in its proper context.

Some tests measure ethanol itself, while others look for substances produced as the body processes alcohol, such as ethyl glucuronide or ethyl sulfate. Metabolite testing can remain reactive after ethanol is no longer present in the bloodstream. A positive result indicates exposure within the test's detection window; it does not establish how much a person drank or how impaired they were at a particular time.

A negative result means the target was not detected above the test's threshold. Timing, diluted urine, the particular assay, and sample handling can all affect interpretation.

Follow the collection instructions exactly. Tell the requesting clinician or testing service about medicines, alcohol-containing mouthwash or remedies, recent drinking, and any difficulty producing a sample. Do not drink excessive water to change the result; over-dilution can make the sample unsuitable and may require repeat collection.

For a medical test, ask which substance is being measured and why. Results from a general urinalysis for infection, blood, glucose, or protein are different from a specific alcohol assay.

Seek emergency help for slow or irregular breathing, inability to wake, a seizure, blue or grey lips, repeated vomiting, severe confusion, or collapse after drinking. Stay with the person, place them on their side if they are breathing but unresponsive, and do not assume they can sleep it off.

Arrange medical support before abruptly stopping alcohol if regular heavy use has led to shaking, sweating, anxiety, hallucinations, or seizures when intake falls. Those features can signal withdrawal that requires supervised care.

Ask whether the test detected ethanol or a metabolite, what the laboratory threshold means, whether confirmation was performed, and whether any medical treatment depends on the result. For workplace or legal testing, obtain interpretation from the responsible testing programme because its collection and reporting rules may differ from clinical care.
