Alcohol and heart failure

Some people with heart failure may be advised to avoid alcohol completely. The right advice depends on the cause and severity of the condition, current symptoms, medicines and whether alcohol has previously weakened the heart.

Alcohol may raise blood pressure, trigger an irregular or faster heartbeat and make it harder to follow fluid or dietary guidance. Heavy or repeated drinking can also directly weaken heart muscle. Heart failure can present with breathlessness, fatigue, cough or swollen ankles; a change in these symptoms may indicate fluid is building up.

Alcohol can cause dizziness or drowsiness that overlaps with medicine effects. It may also lead to missed doses or make it difficult to notice that heart failure is deteriorating. Do not stop or alter prescribed treatment to make room for drinking.

Check your personal alcohol advice with the clinician managing your heart failure. Tell them honestly what and how often you drink, including any episodes of heavy drinking. Ask whether alcohol interacts with your medicines and how it fits with any fluid limit.

If you have been drinking heavily or feel dependent on alcohol, seek medical help before stopping suddenly because withdrawal can be dangerous. Keep taking heart medicines as prescribed and track changes in breathing, ankle swelling, palpitations and ability to do usual activities.

Arrange a prompt clinical review if breathlessness, swelling, coughing, fatigue or palpitations are new or getting worse, or if alcohol is making medicine use difficult. Contact emergency services for severe difficulty breathing, chest pain, fainting, blue or grey lips, sudden confusion, or a rapid deterioration. Do not drive yourself if you are unwell or have been drinking.
