Understanding AJCC melanoma staging

It gives clinicians a consistent way to describe how far melanoma has been found. The stage is built from several results rather than the appearance of a skin lesion alone, and its meaning depends on whether it was assigned before or after definitive surgery.

The primary tumour category reflects features reported by the pathologist, including thickness and ulceration. The regional-node category considers melanoma in nearby lymph nodes and certain deposits close to the original site. The distant-metastasis category records spread beyond the regional area and where it has been detected.

Doctors combine these categories into a stage group. A biopsy can establish melanoma and provide key tumour measurements; an examination, node procedure or imaging may add the remaining information. If new pathology becomes available, the stage description may be clarified without implying that the disease suddenly changed on that day.

Request the complete stage notation, not only the overall group, and ask which reports support it. Your specialist can explain whether surgery has removed the visible primary melanoma, whether lymph-node sampling would add useful information, and whether imaging or systemic therapy belongs in the plan.

Follow-up is tailored to the melanoma findings and treatment received. Attend planned skin and lymph-node examinations, learn how to check your skin between visits, and protect it from further ultraviolet exposure. Bring photographs or a dated note when you notice a change, because comparison can be more informative than memory alone.

Arrange timely review for a new pigmented lesion that is changing, a persistent nodule around a previous melanoma scar, or swelling in a nearby armpit, groin or neck. Also report ongoing unexplained cough, focal bone pain, headaches, balance problems or weakness. Urgent assessment is appropriate for a sudden neurological deficit, a seizure or severe breathing difficulty.

Ask: What are my T, N and M categories? Was my stage assigned clinically or pathologically? Are any results still pending? How does each finding influence the next treatment decision? What should my skin self-check include, and whom should I contact if a new symptom appears?
