Air subcision: understanding scar release treatment

If a clinic uses the word “air,” ask it to identify the device, substance, and exact technique. That description may refer to a branded method or an additional step, and it should not be assumed to be equivalent to established scar subcision.

Some depressed scars are pulled downward by fibrous bands under the skin. In subcision, a clinician passes a suitable needle or other surgical instrument beneath the scar and moves it to divide those attachments. Releasing the tether can allow the surface to lift, while healing beneath the scar may add support.

The procedure is aimed at scars with a tethered component, not every scar shape. Bruising, swelling, tenderness, and temporary firmness can follow. Bleeding, infection, pigment change, a blood collection, altered sensation, and an uneven result are possible. Improvement may be incomplete, and scars can remain visible.

Choose a clinician who can assess scar type, skin condition, medicines, healing history, and tendency to develop pigment changes or raised scars. Ask whether active acne or a skin infection should be controlled before treatment and whether another technique is planned during the same visit.

Use prescribed wound care, keep puncture sites clean, and avoid pressing or manipulating the area unless specifically instructed. Follow the clinic’s guidance on exercise, cosmetics, shaving, and sun protection. Do not copy online needling or scar-release demonstrations; working below the skin can injure vessels, nerves, and deeper structures.

Contact the treating clinician for rapidly expanding swelling, pain that is escalating, spreading redness, pus, fever, persistent bleeding, marked asymmetry, or numbness that is new or worsening. Sudden visual symptoms, difficulty breathing, facial weakness, or severe illness after a facial procedure warrants urgent assessment.

A firm or bruised area may occur during ordinary recovery, but a growing tense lump could represent collected blood and needs prompt examination. Attend scheduled follow-up so the clinician can distinguish normal healing from a complication and decide whether any further scar treatment is suitable.

Request the full procedure name, the clinician’s rationale for your scar pattern, the instrument and anaesthetic used, and an explanation of what “air” means in the clinic’s wording. Ask about expected bruising, pigment change, emergency contact arrangements, and why laser treatment might be combined with or separated from subcision.
