Aggressive melanoma: what the term means

It is not a precise stage by itself. The pathology findings, lymph-node assessment and scans when indicated give the care team more useful information about extent and treatment.

A pathology report describes how deeply the melanoma extends, whether its surface is ulcerated, how actively cells are dividing and whether the edges of the removed tissue contain tumour. These details help determine further surgery and staging.

A sentinel lymph node biopsy may be discussed for selected melanomas to check the first draining lymph nodes. When melanoma has spread beyond the original site, treatment planning may include immunotherapy and other systemic options as well as surgery in suitable situations.

Keep specialist appointments and bring the pathology report, procedure records, medicine list and details of other health conditions. Note any new skin changes or symptoms, but do not delay a scheduled visit while trying to interpret them yourself.

Treatment may begin with wider surgical excision around the original melanoma. The team then tailors additional care to the stage, tumour features, test results, general health and the likely balance of benefit and side effects. Ongoing skin and lymph-node checks remain part of follow-up.

Contact the cancer team promptly about a new lump, unexplained persistent pain, worsening breathlessness, marked weakness or neurological symptoms. A new severe headache, seizure, confusion, fainting, sudden weakness, speech difficulty or loss of coordination needs emergency assessment because brain involvement is one possible cause among several.

A changing mole or a new pigmented lesion that grows, bleeds or develops an irregular shape also merits timely skin examination, whether or not it is close to the original melanoma.

Ask for the exact stage, the pathology features driving the plan, whether lymph-node testing or scans are recommended, the aim of each treatment, likely side effects, who to contact between visits, and the schedule for surveillance. Request plain-language copies of important reports if helpful.
