Age spots on the palm: why diagnosis comes first

Many palm marks are benign, but possibilities include a mole, pigment after irritation, dye or chemical staining, a tiny bleed beneath thick skin, infection, or another pigmented lesion. Calling it an age spot without examination can delay the correct care.

Clinicians consider when the mark appeared, whether it follows a cut or blister, and whether its colour remains within the natural palm lines or crosses them. Magnified examination can reveal pigment patterns that are not visible in an ordinary photograph.

Surface scale or itch may suggest an inflammatory or infectious process, while tenderness after pinching or manual work may fit trapped blood. These clues guide assessment but do not safely identify a pigmented palm lesion at home.

Wash gently to see whether superficial dye lifts, but do not scrape, bleach, burn, or freeze the area. Take a focused photograph with a ruler beside the mark, then repeat in the same lighting if you are waiting for an appointment.

Arrange a clinician-led skin examination for an unexplained mark, particularly if it is new. Bring details of hand injuries, chemical contact, new medicines, similar marks on the soles, and any family or personal history of significant skin lesions.

Seek a prompt dermatology review if the mark enlarges, changes, has an irregular outline, contains several colours, bleeds, cracks repeatedly, becomes a lump, or fails to heal. A dark streak extending into a fingernail also needs assessment.

Get urgent care for rapidly spreading redness, severe pain, pus, fever, loss of feeling, or difficulty moving the hand. These signs raise concern about infection or deeper injury rather than indicating that a dark spot has one specific cause.

Ask what the magnified pattern suggests, whether fungal testing or a small tissue sample is needed, and how the mark will be monitored. If it is confirmed as benign, ask whether removal is worthwhile given that palm procedures may affect grip comfort or leave a scar.
