When Ankylosing Spondylitis Can Begin

The first symptoms may be gradual and easy to dismiss, so the age at diagnosis may not match the age when inflammation began. Age helps a clinician recognise a pattern; it cannot confirm the condition or rule it out on its own.

Ankylosing spondylitis often presents with persistent lower back or buttock pain and morning stiffness that improves with movement yet feels worse after rest. Symptoms may wake a person during the night. Some people also develop pain where tendons attach to bone, swollen peripheral joints, fatigue, psoriasis, bowel inflammation or eye inflammation.

Mechanical back pain is more often linked to a particular movement or load, although patterns can overlap. A clinician considers the whole history, physical examination, blood tests and imaging rather than using onset age as a stand-alone test.

Arrange an assessment when back pain and stiffness persist, recur, interrupt sleep, or improve with activity more than rest. Mention any family history, heel pain, swollen joints, psoriasis, bowel symptoms or episodes of a painful red eye. These details can help determine whether referral to a rheumatologist is appropriate.

If the condition is diagnosed, regular tailored exercise and physiotherapy can support posture, spinal mobility and function. Medicines may reduce pain and inflammation or control inflammatory activity; treatment choice depends on symptom pattern, other health conditions and response to earlier care.

A painful red eye, light sensitivity or blurred vision needs urgent assessment because eye inflammation associated with ankylosing spondylitis can threaten vision if untreated. Sudden severe spinal pain after even a modest injury also warrants prompt evaluation in someone with a stiff or fused spine.

Seek emergency care for new leg weakness, numbness around the groin, loss of bladder or bowel control, severe chest pain or major breathing difficulty. Urgency comes from these warning signs, regardless of the person's age.

Ask whether your pain pattern suggests inflammatory back pain, what alternative causes are being considered, and whether blood tests, imaging or rheumatology referral are needed. If you already have a diagnosis, ask how progress will be monitored and which exercises match your current mobility.
