Bleeding the day after sex: what to know

The timing does not necessarily mean the bleeding began a full day later; a small amount may remain inside the vagina and only become visible later. Note its colour, amount, duration, and whether it happens again.

Friction, limited lubrication, or a small surface injury can cause light spotting. Cervical inflammation, including from an infection such as chlamydia, may also bleed after contact. Around menopause, lower oestrogen can make vaginal tissue thinner and more easily irritated.

Cervical dysplasia, cervical cancer, and vaginal cancer can present with bleeding after sex, but this symptom alone cannot identify the cause. Repeated episodes deserve assessment even when the bleeding is light.

Use a pad or liner so you can monitor the flow, and avoid sex or inserting anything into the vagina until active bleeding and soreness settle. If pregnancy is possible, mention this when arranging care.

Book a clinician visit if the bleeding recurs, continues, follows every sexual encounter, or comes with unusual discharge, pelvic pain, pain during sex, or urinary symptoms. The assessment may include a pregnancy test, infection testing, and examination of the vagina and cervix.

Get urgent medical help for heavy bleeding, fainting, marked dizziness, severe or worsening pelvic pain, fever, or weakness. Seek prompt assessment for bleeding with a positive pregnancy test, especially if pain is one-sided or you feel faint.

Any new vaginal bleeding after menopause should be medically assessed, even if it stops and does not hurt.

Ask whether you need a cervical examination, cervical screening, pregnancy testing, or tests for sexually transmitted infections. Also ask how long to avoid sex, what changes should trigger an earlier review, and how you will receive any test results.
