Constipation after pregnancy: remedies and warning signs

Short answer
After pregnancy, constipation often improves with regular fluids, fibre-rich food, gentle movement and unhurried toilet habits; a clinician or pharmacist can recommend a suitable laxative when these measures are not enough.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Helpful first measures
- Regular fluids, gradually increased fibre, gentle walking and a supported toilet position
- Medicine check
- Iron and some pain medicines may contribute; discuss changes with the prescriber
- Urgent signs
- Severe pain, repeated vomiting, marked swelling, inability to pass gas, heavy bleeding or fainting
The short answer
Avoid straining, especially while a perineal tear, episiotomy or caesarean wound is healing. Support your feet on a low stool, lean forward and breathe out gently rather than holding your breath.
Why bowel movements may be difficult after birth
Reduced movement, drinking less than your body needs, changes in meals and fear of pain around stitches can all slow or delay a bowel movement. Iron supplements and some pain medicines can also make stools harder.
Constipation can involve hard or lumpy stool, straining, fewer bowel movements than usual, or a feeling that the bowel has not emptied. Passing stool does not normally open correctly repaired stitches, although discomfort can make the first bowel movements feel worrying.
Steps that can make stool easier to pass
Sip fluids regularly and use urine colour as a practical guide: dark urine or a dry mouth may suggest you need more fluid. Increase fibre gradually through vegetables, fruit, pulses and whole grains; a sudden large increase can worsen bloating.
Try sitting on the toilet after a meal, when the bowel is naturally more active. Do not ignore the urge to go. Short, comfortable walks can help bowel movement if your recovery plan allows them.
If self-care is not helping, ask a pharmacist, midwife or doctor about a stool softener or an osmotic laxative. Mention breastfeeding, current medicines, kidney or heart problems, and any tear or surgery. Do not stop prescribed iron or pain relief on your own; the prescriber may be able to adjust the medicine or add bowel support.
When to seek medical care
Seek urgent medical assessment for severe or worsening abdominal pain, repeated vomiting, marked abdominal swelling, inability to pass gas, heavy rectal bleeding, fainting, or fever with feeling very unwell. These signs need assessment rather than repeated home remedies.
Contact your clinician promptly if pain prevents you from passing stool, bleeding keeps recurring, the wound becomes more painful or starts discharging, or constipation continues despite fluids, fibre and an appropriate laxative. A review can check medicines, hydration, haemorrhoids, pelvic-floor problems and less common underlying causes.
Useful questions for your clinician
Ask whether any prescribed iron or pain medicine could be contributing, which laxative fits your health history and feeding plan, and how long to use it. Also describe stool consistency, abdominal symptoms, rectal bleeding, wound pain and what you have already tried.

Questions people ask
Will passing stool damage stitches after childbirth?
A bowel movement does not normally open a correctly repaired perineal wound. Avoid forceful straining, use a footstool for support, and seek care if wound pain is worsening or there is discharge.
Can I take a laxative while breastfeeding?
Some laxatives may be suitable during breastfeeding, but the choice depends on the product and your medical history. Check with a pharmacist, midwife or doctor before starting one, and follow the recommended dose.
Should I stop iron tablets if they make me constipated?
Do not stop prescribed iron without speaking to the prescriber. They can review why you need it, consider a different preparation or schedule, and recommend bowel measures that fit your recovery.
