Bleeding after sex just after a period

Friction, vaginal dryness, inflammation, or changes on the cervix can also cause bleeding with contact. The colour or small volume cannot reliably identify the source.

Sex can release blood that remained in the vagina as menstruation ended. Dry or delicate vaginal tissue can bleed after friction, especially around menopause; genitourinary syndrome of menopause can present with dryness, burning, discomfort during sex, and contact bleeding.

Chlamydia can present with bleeding after sex, unusual discharge, pelvic discomfort, or no noticeable symptoms. Cervical dysplasia and cervical cancer can present with contact bleeding, while vaginal cancer can present with bleeding that is not part of a usual period. These possibilities do not mean that bleeding identifies a specific condition; examination and appropriate testing establish the cause.

Note whether the bleeding followed penetration, its colour and amount, whether it stopped, and any pain, dryness, discharge, odour, urinary symptoms, or pregnancy possibility. Avoid further penetration if it is painful or restarts bleeding; a water-based lubricant may reduce friction when dryness is the likely trigger.

Arrange an appointment if bleeding happens again, occurs away from a period, follows menopause, or comes with discharge or pelvic pain. A clinician may inspect the vagina and cervix, offer infection testing, review cervical screening history, and decide whether further investigation is needed.

Seek urgent help for heavy or continuing bleeding, severe lower abdominal pain, fainting, marked dizziness, fever, or repeated vomiting. Bleeding with significant pain and a positive pregnancy test or possible pregnancy also needs prompt assessment.

If you feel weak, confused, short of breath, or close to collapse, call emergency services rather than travelling alone.

Ask whether the bleeding appears to come from the vagina or cervix, whether infection testing is appropriate, and whether cervical screening is due. Explain any new partner, dryness, painful sex, contraception, medicines that affect bleeding, and bleeding after menopause.
