Ovulation pain, also called mittelschmerz, is often one-sided and may feel like a dull ache, pressure, or brief sharp sensation. Pain after the suspected ovulation day might still be cycle-related because calendar estimates do not show the exact moment an egg was released.

Endometriosis can present with recurring pelvic pain, painful periods, pain during sex, or bowel and bladder discomfort. Ovarian cysts can present with pelvic pressure or one-sided pain.
