What to expect after a natural birth

Short answer
After a natural birth, expect vaginal bleeding, cramps, perineal soreness, tiredness, and gradual changes in feeding, bladder, bowel, and emotional wellbeing.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common early changes
- Bleeding, cramps, soreness, fatigue, and breast fullness
- Helpful basics
- Rest, fluids, nourishing food, hygiene, and gentle movement
- Emergency signs
- Very heavy bleeding, fainting, chest pain, breathing difficulty, seizure, or dangerous thoughts
The short answer
Recovery differs from person to person and may be affected by stitches, assisted delivery, blood loss, sleep, and feeding. Your maternity team should explain what was found during birth and how to care for any tear or wound.
Changes during recovery
Vaginal discharge after birth starts bloody and should gradually become lighter. Cramp-like afterpains happen as the uterus contracts; they may feel stronger during breastfeeding. The area between the vagina and anus can be bruised or sore, especially after a tear, stitches, or an assisted birth.
Passing urine may sting, bowel movements can feel daunting, and pelvic-floor weakness may cause leakage with coughing or movement. Breasts can become full or tender as milk production changes. Tearfulness, irritability, or feeling overwhelmed can occur, but persistent low mood, severe anxiety, frightening thoughts, or difficulty functioning deserves support.
How to support healing
Rest when you can, accept practical help, drink regularly, and choose fibre-rich meals. Change maternity pads often, wash the perineal area gently, pat it dry, and follow the instructions given for stitches. Begin gentle walking and pelvic-floor squeezes when comfortable, then increase activity gradually rather than pushing through heavier bleeding or pain.
Use only pain relief that your clinician or pharmacist says is suitable for you, particularly if you are breastfeeding or taking other medicines. For feeding pain, poor latch, cracked nipples, or concerns about milk transfer, seek early breastfeeding support. Keep postnatal appointments even when recovery seems straightforward.
When to get medical help
Seek emergency help for sudden very heavy bleeding, fainting, chest pain, trouble breathing, a seizure, confusion, or thoughts of harming yourself or the baby. A severe or persistent headache, especially with vision changes or vomiting, also needs urgent assessment after birth.
Contact your maternity team promptly for fever or chills, worsening abdominal or perineal pain, unpleasant-smelling discharge, difficulty passing urine, a red or draining wound, or one-sided calf pain, redness, or swelling. A hot, red, painful breast with flu-like illness should also be assessed.
Questions for your maternity team
Ask whether you had a tear and how it was repaired, which pain medicines suit feeding and your medical history, and when to restart driving, exercise, sex, or contraception. You can also ask how to distinguish expected bleeding from concerning bleeding, where to call outside clinic hours, and who can help with feeding or pelvic-floor symptoms.

Questions people ask
Is bleeding expected after a natural birth?
Yes. Post-birth discharge commonly begins bloody and should trend lighter. Get urgent help if bleeding suddenly becomes very heavy or makes you faint or dizzy.
Can I start exercising after delivery?
Gentle walking and comfortable pelvic-floor exercises may begin early. Build up slowly and pause if activity increases pain, pressure, or bleeding; ask your clinician about recovery after a significant tear or other complication.
When should emotional changes concern me?
Seek support when low mood, anxiety, or distress persists, worsens, or interferes with sleep and daily care. Confusion, hallucinations, or thoughts of harm require emergency help.
