When to expect a period after miscarriage

Your first menstrual bleed may not match your former calendar or usual flow. Loss-related bleeding should first taper and stop. A later, distinct episode is more likely to be menstruation, but ongoing spotting can make that distinction difficult.

Hormone levels can take longer to settle when the pregnancy was further advanced or pregnancy tissue remains. Medical or surgical treatment, breastfeeding, previously irregular cycles, stress, thyroid conditions, and polycystic ovary syndrome can also affect the next ovulation.

Ovulation is silent for many people and comes before bleeding. If unprotected sex occurs, an absent expected period may represent a new pregnancy as well as a cycle that has not yet restarted.

Keep a simple record of bleeding days, pad changes, clots, pelvic pain, discharge, and test results. Follow your team's instructions for a home pregnancy test, and report a result that stays positive or turns positive again after becoming negative.

Arrange an appointment if menstruation has not returned within the timeframe your clinician gave you, or if spotting continues and you cannot identify a break between miscarriage bleeding and a possible period. Assessment can include a symptom review, examination, hormone testing, or ultrasound.

Prompt emergency assessment is needed for bleeding that quickly soaks pads, collapse or faintness, intense or worsening pelvic pain, pain at the shoulder tip, fever, foul discharge, or a strong sense that you are seriously unwell. These features can signal blood loss, infection, retained tissue, or ectopic pregnancy and should be assessed regardless of the calendar.

Ask what date to use as the start of recovery, how long to monitor before contacting the service, and whether a negative pregnancy test is expected. Share whether you plan to conceive or prevent pregnancy, since ovulation may occur without warning and contraception may be needed before a period.
