Bleeding after menopause is not a period
Menopause marks the end of menstrual cycles after the ovaries stop releasing eggs regularly and hormone levels change. Bleeding later may resemble a period, but it has another source.
What could cause period-like bleeding
Possible explanations include fragile vaginal tissue related to lower oestrogen, polyps, inflammation, infection, medicines or thickening of the womb lining. Bleeding may also arise from the cervix, urinary tract or rectum.
Cancers of the womb or cervix can present with bleeding after menopause. This is a reason to investigate the symptom, not a conclusion about its cause.
What to do if you bleed
Contact a clinician even if the blood is light, brown, painless or seen only once. Note when it started, how much there was, whether intercourse preceded it and whether you noticed pain, discharge, urinary symptoms or bowel changes.
Bring a list of medicines and hormonal products. The assessment may involve a pelvic examination, ultrasound, inspection of the cervix or a sample of the womb lining, depending on your history and examination findings.
Signs requiring urgent help
Seek urgent help for heavy bleeding, faintness, severe or escalating pain, marked weakness or trouble breathing. Fever or foul-smelling discharge with pelvic pain also needs prompt advice.
When bleeding is slight and you feel otherwise well, book a medical assessment rather than treating it as a returning cycle or waiting to see whether it happens again.
