The short answer

The affected breast may produce less for a while, and pumping output alone does not show exactly how much milk a baby can remove. Recovery varies with healing, feeding effectiveness, and how often milk is removed.

Why output may fall

Mastitis is inflammation of breast tissue. Swelling can narrow milk pathways, tenderness may shorten feeds, and feeling ill can make feeding or expressing harder. If milk is removed less often or less effectively, the breast receives less stimulation to keep producing at its previous level.

A softer breast or a smaller pumped volume does not by itself prove that a baby is underfed. Signs that matter include swallowing during feeds, alertness, wet nappies, weight progress, and whether the baby settles appropriately after feeding. A clinician or lactation professional can assess these together.

How to support feeding and recovery

Continue breastfeeding if you can do so comfortably, or express enough to meet your baby's needs and maintain milk removal. Feed responsively rather than trying to forcefully empty the breast. Check positioning and attachment, especially if feeds remain painful or the nipple is damaged.

Use cool packs between feeds for comfort, rest, drink according to thirst, and take prescribed treatment as directed. Avoid deep breast massage, aggressive squeezing, or adding frequent pumping sessions without individual guidance, because extra pressure and overstimulation may worsen swelling. If supplementation is advised, request a plan that also protects milk production.

When medical review is needed

Seek prompt medical advice if fever, flu-like symptoms, redness, swelling, or breast pain is worsening or not beginning to improve with home care. A persistent or enlarging lump, pus or unusual nipple discharge, recurrent mastitis, or symptoms after starting prescribed treatment also warrant reassessment. A collection of pus may need examination and drainage.

Get urgent help if you feel severely unwell, faint, confused, short of breath, or unable to keep fluids down. Arrange a feeding assessment if the baby is unusually sleepy, feeds poorly, has fewer wet nappies than expected, or there are concerns about weight gain.
