After fractional CO2 laser treatment: skin care and warning signs

Short answer

Redness, warmth, swelling, sensitivity and fine crusting can occur after fractional CO2 laser treatment as the skin barrier repairs.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Treatment effect
Fractional columns initiate repair while intervening skin supports healing.
Barrier care
Gentle cleansing and the recommended bland moisturiser help protect recovering skin.
Avoid
Do not pick crusts or restart irritating products too early.

The short answer

The surface may feel rough, dry or tight before it sheds naturally. Recovery varies with treatment intensity, the site and individual skin characteristics. Use the exact cleansing, moisturising and sun-protection plan supplied by your clinician, since recently treated skin is unusually vulnerable to irritation, infection and pigment change.

Why the skin reacts

A fractional CO2 device creates narrow treatment columns while leaving intervening skin intact. The controlled injury starts repair and remodelling, but also temporarily weakens the protective barrier. Redness and swelling reflect inflammation; pinpoint crusts or a grid-like appearance can match the fractional delivery pattern.

Skin tone can look darker or lighter during recovery. Persistent pigment change is more concerning than brief post-treatment redness, especially after unprotected sun exposure or irritation from unsuitable skincare.

Protect the treated skin barrier

Wash gently with the cleanser and water temperature advised by your clinic, pat rather than rub, and apply the recommended bland ointment or moisturiser with clean hands. Do not pull off flakes or crusts. Avoid scrubs, exfoliating acids, retinoids, fragranced products and hair-removal methods on the area until your clinician says the barrier has recovered.

Limit heat, heavy sweating and friction while the skin is acutely inflamed. Once sunscreen is permitted, use broad-spectrum protection and physical shade consistently. If you have a history of cold sores, take any prescribed antiviral medicine exactly as directed and report tingling or grouped blisters early.

When recovery needs assessment

Contact the treating clinician promptly if pain or swelling is intensifying, redness spreads beyond the treated area, pus or an unpleasant smell develops, fever occurs, or grouped blisters appear. Grey, dusky or very pale patches, open sores, eye pain, vision changes or a burn extending beyond the intended site also require urgent review.

Seek emergency help for breathing difficulty or swelling of the mouth or throat. Photographing a changing area in consistent light can help the clinic understand its progression, but should not delay care.

Questions before resuming your routine

Ask when you may restart makeup, exercise, shaving and active skincare, and which moisturiser and sunscreen suit your treated skin. Confirm how to distinguish expected crusting from infection, when the next review is due and whether your tendency to pigmentation or cold sores changes the aftercare plan.

Questions people ask

Is a grid pattern normal after fractional CO2 laser treatment?

A temporary dotted or grid-like pattern can reflect how fractional energy was delivered. Ask the clinic to review it if it becomes increasingly painful, blistered, discoloured or weepy.

When can I use retinol or exfoliating acids again?

Wait until the barrier has healed and your treating clinician confirms it is appropriate. Restarting too soon can increase irritation and pigment disturbance.

Should I peel off flaking skin?

No. Allow flakes and fine crusts to release naturally, because pulling them away can injure the new surface and raise the chance of infection or marks.
