Leg swelling after delivery: what is expected and what is urgent

Pregnancy-related fluid, intravenous fluids and reduced movement can leave feet, ankles and lower legs puffy. The swelling should be considered alongside pain, redness, blood pressure symptoms, chest symptoms and whether one leg looks different from the other.

The body gradually clears fluid retained during pregnancy and fluid received around labour or surgery. Gravity draws some of it into the lower legs, particularly after sitting or standing, and normal circulation changes take time to reverse.

Swelling on both sides without marked pain can fit this fluid shift. A blood clot more often presents with swelling, pain, warmth or colour change affecting one leg, although symptoms vary and examination is needed when a clot is suspected.

Take gentle walks as advised after delivery, change position regularly and elevate the legs when resting. Wear loose clothing and comfortable shoes, drink according to thirst, and continue prescribed blood-thinning medicine exactly as directed.

Do not massage a leg that is newly painful, warm, red or much more swollen than the other. Avoid taking water tablets or herbal products without clinical advice because the correct approach depends on blood pressure, kidney function, heart symptoms, feeding and other medicines.

Seek emergency help for sudden breathlessness, chest pain, coughing blood, collapse, a racing heartbeat with severe weakness, or new one-sided leg swelling and pain. These symptoms can accompany a clot and should not wait for a routine appointment.

Urgent maternity assessment is also appropriate for a severe headache, vision changes, pain under the ribs, vomiting, sudden swelling of the face or hands, or feeling acutely unwell because blood-pressure complications can occur after birth. Fever, spreading redness or fluid leaking from a wound also needs prompt review.

Describe when swelling began, whether it is equal on both sides, and any pain, warmth, breathlessness, headache or vision change. Ask whether your blood pressure, urine, heart, kidneys or leg veins need checking and what level of activity is suitable after your delivery.
