What to do after coughing blood in sputum

A small streak can come from irritated airways after forceful coughing, but infection and ongoing lung conditions can also cause it. The amount, recurrence and accompanying symptoms determine how urgently you need care; the appearance alone cannot confirm the cause.

Blood mixed with phlegm may arise from the nose, throat or breathing tubes. Hard coughing can break tiny surface vessels. Acute bronchitis and pneumonia can present with blood-streaked sputum, while bronchiectasis and COPD may cause recurrent episodes.

Blood vomited from the stomach may look darker or resemble coffee grounds and is a different problem. Blood from the airways is commonly brought up with a cough and may be mixed with mucus. If you cannot tell where it came from, treat it as needing prompt assessment.

Contact a healthcare professional promptly, even if the bleeding stops. Note whether the blood was a streak, clot or larger amount; its colour; how often it happened; and whether fever, chest pain, breathlessness, weight loss or a recent nosebleed occurred.

Bring a list of medicines, especially blood-thinning treatment, and describe smoking or vaping exposure and existing lung disease. Do not take leftover antibiotics: treatment depends on the cause, and not every episode results from a bacterial infection.

Call emergency services if you cough up more than small streaks, bleeding continues or increases, breathing is difficult, chest pain develops, you feel faint or confused, or your lips look blue or grey. Sit upright if comfortable and do not drive yourself.

Prompt same-day care is appropriate for repeated small amounts, fever, worsening cough, unexplained weight loss, or an episode while taking a blood thinner. New calf swelling or sudden breathlessness with bloody sputum also requires emergency assessment.

Ask which source of bleeding is suspected, whether you need chest imaging or sputum testing, and whether any current medicine needs adjustment. Confirm which changes should send you directly to emergency care and when persistent cough or phlegm should be reviewed again.
